We help individuals remain safely and comfortably in their homes for as long as they choose, while empowering them to Make Each Day Count.
In this role, you will provide high-quality nursing care in clients' home environments, helping them achieve the best possible health outcomes while maintaining their independence and dignity.
Safely assist clients with mobility and transfers using mechanical lifts (Hoyer, sit-to-stand) or manual assistance, following prescribed mobility plans and ensuring proper body mechanics and safety procedures.

We are currently seeking compassionate, self-motivated, and skilled Community Nurses to support ALC (Alternate Level of Care) clients throughout the Kawartha Lakes and Haliburton Region.
Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Must be able to communicate effectively in English, both orally and in writing, for documentation and communication with clients and care teams.
Must be comfortable and capable of providing care to both male and female clients of varying ages and care needs.
Must have a valid driver's licence and reliable access to vehicle to complete daily tasks Ability to identify and report client care concerns to Care Coordinators when needed.
