About the role
- This role is employed by Phoenix Med Health (MSO), a healthcare management organization supporting medical practices across operations, growth, and technology.
- You will work directly on behalf of Active Vital Care (www.activevitalcare.com), a mobile, multi-specialty medical provider delivering in-home care to patients across assisted living facilities, group homes, and private residences.
- This is a growth-focused role responsible for driving patient volume through strong, consistent relationships.
Responsibilities
- Drive consistent patient referrals and new patient registrations
- Conduct regular in-person visits to partners
- Identify and onboard new referral partners
- Resolve issues quickly and maintain strong partner satisfaction
- You build trust quickly with both clinical and administrative stakeholders
- You follow up relentlessly and own your pipeline
- Our teams work directly with partner practices across operations, revenue cycle, business development, and infrastructure-driving real, measurable outcomes in the field.
Requirements
- You know how to turn relationships into referrals
Nice to have
- Existing relationships with local facilities or agencies
- Experience with CRM tools and EMRs
- Understanding of the Medicare / senior care landscape
- Monthly new patient volume
- Retention and engagement of key accounts
