About the role
- This provider will engage as a member of a collaborative team and may be assigned to work with specific populations such as TY, forensic or dual.
- This provider will also be responsible to coordinating with outside agencies to coordinate services such as OOD.

Requirements
- Drug Screen, TB Test, Background check (required)
- Must be comfortable making home visits throughout the community and transporting clients in own car.
- Must have an acceptable driving record (4 points or less), reliable transportation, and carry automobile insurance with minimum liability limits of $100,000/$300,000 and property damage of $50,000. (required)
- Three years related experience with High School Diploma or equivalent, or Associate's degree
