About the role

  • The PACE Clinic Assistant provides nursing education to participants, caregivers, and PACE staff.
  • The PACE Clinic Assistant provides case management and care coordination during care transitions and proactively communicates with hospital and long-term care facility case managers regarding participant discharge care plans.
  • The PACE Clinic Assistant is responsible for scheduling participant appointments for PACE Clinic referrals made to medical specialists and ancillary care providers in the community.

Requirements

  • Relevant experience must be within the last 3 years.
