About the role
- Conduct initial screenings and gather information to identify family strengths, needs, and goals.
- Provide ongoing follow-up, monitor progress toward goals, and adapt care plans based on evolving family needs.
- Demonstrate flexibility and adaptability in supporting families across diverse community settings.
Responsibilities
- Collaborate with clinicians and families to develop and update individualized care plans that align with family-identified priorities.
- Actively participate in High Fidelity Wraparound training and apply wraparound principles, while also providing direct support and intervention beyond facilitation.
- Maintain accurate documentation and uphold the highest standards of ethics, confidentiality, and cultural responsiveness.

Requirements
- Education: Bachelor's degree in Social Work, Psychology, Human Services, or related field (required).
- Training: Will receive specialized training and coaching in High Fidelity Wraparound upon hire.
- Language Skills: Bilingual (Spanish) preferred, depending on location.
- Other Requirements: Valid driver's license, auto insurance, and willingness to travel to homes and community settings.
- Requires living in or near the location of the posting.

- Type: Full-Time
- Schedule: Flexible, with required availability for 2-3 evenings per week (4-7pm) or 3-4 weekend hours.
Nice to have
- Prior experience in care coordination, case management, or working with children, youth, and families preferred.

Compensation
- $26.44 - $31.25 per hour
