Alder is seeking a dedicated, clinically licensed, and systems-savvy Care Manager to pioneer and grow our dedicated I/DD practice.

Holistic Care Management: Develop, implement, and oversee comprehensive, person-centered plans tailored to the unique goals, clinical needs, and long-term milestones of the individual.
Transition & Lifespan Support: Provide expert guidance during critical milestones, including school-to-adult transitions, residential placement planning, employment changes, and future-care planning.
Precise Documentation: Maintain accurate, timely, and thorough documentation of all care planning, clinical advocacy efforts, and family interactions.
Historically rooted in aging life care management, Alder is expanding its premier, private-pay advocacy and care coordination model to support individuals with intellectual and developmental disabilities (I/DD) and their families across the lifespan.
Our Care Managers are seasoned clinicians who provide both strategic direction and hands-on support.
Develop, implement, and oversee comprehensive, person-centered plans tailored to the unique goals, clinical needs, and long-term milestones of the individual.
Maintain accurate, timely, and thorough documentation of all care planning, clinical advocacy efforts, and family interactions.
Exceptional interpersonal, writing, and diplomatic skills, with the ability to build immediate trust with families experiencing high stress.

Master's degree in Social Work (MSW) with an active LCSW or LICSW.
Master's degree in Counseling or Psychology with an active LMHC.
Required skills / experiences: Licensure & Credentials: Must possess at least one of the following active Massachusetts credentials: Master's degree in Social Work (MSW) with an active LCSW or LICSW.

In this role, you will act as a strategic partner for individuals with developmental disabilities, autism spectrum disorders, and complex medical or mental health needs.
