About the role
- The FCM plays a key role in coordinating care and managing transitions within the Ohana system of care, including admissions, discharges, and movement between levels of treatment.
- The Family Care Manager job family consists of two levels of professional, non-management roles: Unlicensed Family Care Manager and Licensed Family Care Manager.
- Will complete mandatory 5150 training by Monterey County to obtain 5150 certification and obtain 5150 Professional staff authorization yearly.
Responsibilities
- The FCM also facilitates referrals to appropriate community-based services and programs outside of Ohana when needed and maintains knowledge of effective local and regional resources to support families.
- In this role, the FCM builds collaborative relationships with external providers and organizations while also providing psychoeducation through individual support, groups, and workshops.
- The FCM also performs additional duties as assigned to support high-quality patient care.
- Team members maintain an unwavering commitment to family centered care and delivering quality outcomes.
Requirements
- Master's degree in Social Work is required.
- Effective 10/16/2023 and Associate registration with the BBS at the time of application is required.
- Candidates must have strong interpersonal skills, communication skills, flexibility, curiosity, high energy, dedication and humility.
Compensation
- Hourly Evening Shift Differential: $3.39
- Hourly Night Shift Differential: $5.09
