- Job Description: SCOPE: The Survivorship Coordinator is a clinically active role responsible for the hands-on implementation and day-to-day operations of the cancer survivorship program at Mary Bird Perkins Cancer Center.
Responsibilities
- Mary Bird Perkins and its partners work together to provide state-of-the-art treatments and unparalleled collaborative, comprehensive cancer services.

- Survivorship Care Plan Completion and Coordination
- Other duties as assigned
- High school diploma or GED required.
- Active LPN licensure in Louisiana preferred.
- Minimum 2 years of clinical experience as an MA or LPN, preferably in oncology, primary care, or chronic disease management.
- Experience with chart preparation, patient intake, and EMR documentation required.
- Prior experience in a survivorship, care coordination, or navigation setting strongly preferred.
- Familiarity with oncology workflows and multidisciplinary care teams a plus.
