About the role

  • This role may be full-time or part-time depending on market needs.
  • At Matter Health, our APPs are more than providers - they are Panel Owners.
  • This is a fully community-based role with no traditional clinic home base.

Responsibilities

  • You will manage a dedicated panel of older adults, serving as their longitudinal primary care provider while building trusted relationships directly within the communities you serve.
  • Basic Life Support (BLS) certification

Requirements

  • Experience with EHR documentation (athenahealth experience preferred)
