Responsibilities
- Develop and revise individualized plans of care based on patient status and evolving needs.
- Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
- Provide counseling and support to patients and caregivers before, during, and after treatment.
Requirements
- Preferably 2+ years of nursing experience, with a focus on geriatrics.
- Nursing degree from an accredited institution.

Compensation
- $72K-90K Annual Salary Comprehensive benefits package including 401(k) match, HMO & PPO Plans, Dental, Vision, Life Insurance and Flexible Spending Account.
- Separate sick time bank.
- Position Details: Main Office location: Exeter, NH Service Area: Exeter, Newfields, Stratham, Hampton falls, Seabrook, South Hampton, Kensington areas.
