About the role
- include providing anticoagulation dosing guidance (e.g., warfarin and heparin), evaluating quality outcomes, identifying gaps in care, and standardizing education, competencies, and best practices across sites.

- While based at a primary site, this role collaborates across facilities, working closely with pharmacists, physicians, nurses, interdisciplinary care teams, and enterprise pharmacy committees.
Requirements
- PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy.
- ability to lift up to 30 lbs.
- Must have ability to clearly communicate verbally by phone or in person
- Follows standard precautions using personal protective equipment as required.
- Join the Cleveland Clinic Florida Research and Innovation Center (FRIC) to gain hands-on research experience with the latest, evidence-based and advanced technologies.
Nice to have
- Board certification.
- Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience.
- Prior experience in critical care, pain management, anticoagulation consult services or opioid stewardship (for opioid role)
- Strong background in acute care/inpatient settings
- Open to candidates without residency if significant
