About the role

  • Daily, Care Managers meet with residents, connect them to vital resources, and ensure that each resident's care plan remains cohesive and supportive throughout our organization, from entry to graduation and beyond.
  • Our Care Managers are a vital part of our continuum of care to minimize relational transitions for residents as they move through our two-step residential program model.
  • Care Managers also provide case management and services for several families, which include expectant mothers with a child up to age 5.

Responsibilities

  • P rovide individual c are coor d i n a t ion for women receiving services and support from Harvest Home who are pregnant, recently postpartum, graduates (new mothers who have recently transitioned into housing), and their children (newborns to age 5 for those in our Harvest Home Family Program).
  • Documentation u s ing databases and systems to input, maintain, and organize services given and need ed in a timely, organized, detailed, and accurate manner.
  • Advocate on resident's and alumnae's behalf for support services in decision-making and self-advocacy.
  • Provide financial assistance, resources, and support with budgeting, financial growth and collecting program fees, etc.
  • Collaborate with the R esidential Life Team on the provision of classes, workshops, groups, and events.

Requirements

  • Must be able to lift 40 lbs.

Compensation

  • Flexibility needed to support one weekend shift per month for Alumnae-based activities (minimum of 3 times per year).
