About the role
- The expected salary range for this position is based on several factors including market, funding, candidates experience level, skills, training, and related knowledge.
- The Waivered Services Case Manager assists clients on a waiver to access needed waiver and State plan services, as well as needed medical, social, educational and other services, regardless of the funding source.
- Goal development and service planning utilizes Person-Centered principles, and incorporates both what is important to and important for the client.
Responsibilities
- Consult with departmental staff, other county departments and community service professionals to develop and provide appropriate and timely services.
- Maintain accurate case records and complete forms for documentation of eligibility, treatment cost reimbursements, service referrals, evaluation of client outcomes and general case management purposes.
Requirements
- The Case Manager also provides ongoing monitoring of services, and serves as an advocate for the client when assistance is needed or services are not being performed as required.
