About the role

  • Conduct initial screenings and gather information to identify family strengths, needs, and goals.
  • Provide ongoing follow-up, monitor progress toward goals, and adapt care plans based on evolving family needs.
  • Demonstrate flexibility and adaptability in supporting families across diverse community settings.

Responsibilities

  • Collaborate with clinicians and families to develop and update individualized care plans that align with family-identified priorities.
  • Actively participate in High Fidelity Wraparound training and apply wraparound principles, while also providing direct support and intervention beyond facilitation.
  • Maintain accurate documentation and uphold the highest standards of ethics, confidentiality, and cultural responsiveness.

Requirements

  • Education: Bachelor's degree in Social Work, Psychology, Human Services, or related field (required).
  • Training: Will receive specialized training and coaching in High Fidelity Wraparound upon hire.
  • Language Skills: Bilingual (Spanish) preferred, depending on location.
  • Other Requirements: Valid driver's license, auto insurance, and willingness to travel to homes and community settings.
  • Requires living in or near the location of the posting.
  • Job Details
  • Type: Full-Time
  • Schedule: Flexible, with required availability for 2-3 evenings per week (4-7pm) or 3-4 weekend hours.

  • Prior experience in care coordination, case management, or working with children, youth, and families preferred.

Compensation

  • $26.44 - $31.25 per hour
