Responsibilities
- Directly manage and develop current Outreach and Enrollment Supervisors (2) and subsequent supervisors for each new center that is launched.
- Manage and develop Ops Quality and Excellence department to capture learnings across centers, provide resources, skill training, and maintain accountability for successful community outreach, engagement of new prospects, and enrollment opportunities.
- Implement sustainable growth operations at the center level, including referral engagement, community outreach, prospective participant case management, and participant handoffs to the PACE center interdisciplinary team.
- Work across the Central Growth Team to develop and implement new programmatic strategies to improve the performance and processes of the Outreach and Enrollment teams
- Collaborate with Central Growth Team to manage and develop coordination and handoffs with centrally run growth programs and large-scale partners (e.g., Kaiser Permanente)
- Regularly review team performance data, identify gaps or bottlenecks, and implement solutions in partnership with local and central teams.
- Partner closely with the Central Growth Team to develop the playbook for growth-focused operational execution at Habitat-developing systems and standards that can scale as we expand to new markets.
- Contribute to the future structure of the growth function and local Outreach and Enrollment Teams, including identifying internal leaders, shaping team design, and evolving operating models.
- Perform tasks that may involve bending, reaching, lifting, or assisting participants, depending on role responsibilities.

Requirements
- Experience making strategic decisions based on P&L performance, with a clear impact on business outcomes.
- Proof of valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law.
- For COVID-19 and influenza, team members who are working in a participant care setting are required to provide either proof of vaccination or a signed declination form and comply with applicable masking requirements while in a care setting to the extent permitted by law in the applicable jurisdiction.
- As a condition of employment, team members are expected to comply with all applicable infection control protocols, personal protective equipment (PPE) requirements, and vaccination requirements associated with their role and work location, and to the extent required by law and as permitted in the applicable jurisdiction.

- Familiarity with the PACE program or other elderly care organizations.
- Experience working in geriatric primary care.

Compensation
- The anticipated base salary range for this position is $173,000 - $205,000. This position is also eligible for a target annual bonus of 15% and may be eligible for additional incentive compensation, as applicable.
- The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
