About the role

  • As a Case Management Social Worker, you will be responsible for coordinating the care and services of select patients.
  • This includes conducting comprehensive psychosocial assessments, providing supportive counseling and collaborating with community resources to address patient needs.

Responsibilities

  • Manage all aspects of discharge planning for assigned patients in order to ensure a timely discharge and provide appropriate connection with post-discharge care providers.

Requirements

  • Master's degree in social work (MSW) or Master of Science in Social Administration (MSSA)
  • Ability to communicate and exchange accurate information
  • Ability to work with physical records, such as retrieving and filing.
  • Ability to operate a computer and other office equipment.
  • Ability to travel throughout the hospital system.
  • Follows Standard Precautions using personal protective equipment as required for procedures.

Nice to have

  • Certification as a Case Manager within one year of eligibility from an approved professional organization
  • Care Management/Social Work experience
  • Knowledge of community resources
  • Detail oriented with ability to work independently within the structure of Cleveland Clinic
  • Ability to grow, nurture and sustain professional relationships with clinical staff
  • Ability to discern when a patient or CG is in emotional distress
