Therapeutic Interventions - Implement individualized treatment plans including therapeutic exercise, gait training, balance activities, and functional mobility tasks under the supervision of the PT.
Participant Assessment Support - Observe, document, and report participant progress, functional changes, and safety concerns to the supervising PT.

Equipment & Safety - Train participants and caregivers on safe use of assistive devices and mobility equipment; maintain a safe therapeutic environment.

Ability to work independently, travel locally, and adapt to changing participant needs.
Accredited PTA Program graduate with an active California PTA license.
Current CPR/BLS certification.
Experience working with geriatric populations or in long ‑ term care, home health, or PACE preferred.
Strong communication, documentation, and teamwork skills.
Schedule & Work Environment
Per diem / as ‑ needed typically 8am to 5pm
Services delivered across Habitat Health PACE Centers (Sacramento, South LA, San Leandro, West Covina) participant homes.
Collaborative, mission driven environment focused on supporting older adults to remain safely in their homes.

The anticipated base salary range for this position is $40.00 - $46.00 per hour.
The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
