About the role

  • As a core member of the integrated care team, alongside the dialysis team, nephrologists, and social workers, you'll see patients in person and virtually.

Responsibilities

  • Clinician Lead - Our executive team has two clinician leaders, so the clinical voice is the top voice in the room.
  • Develop individualized care plans and review them collaboratively with nephrology and the care team.
  • Facilitate referrals to evidence-based psychosocial services when clinically indicated (e.g., grief support, psychiatric providers, medication-assisted treatment).

Requirements

  • Bilingual English/Spanish required | Licensed Social Worker - Professional Counselor - Marriage and Family Therapist

Compensation

  • $73,500 - $112,500 annually
  • Salary: $73,500 - $112,500 annually
