About the role

Oversees data collection, chart abstraction and reporting activities as necessary.
Responsible for Quality measures identification and reporting.
Valid nursing license (RN) in good standing with state licensing board.

Leads root cause analyses.
Leads the implementation and monitoring of quality, patient safety and performance improvement programs.

Minimum of five (5) years' experience in Nursing and minimum of two (2) years of experience in Quality Assessment, Patient Safety or Performance Improvement, required.
Knowledge of and experience with data, analysis and reporting.
Experience with EMR/EHR systems.

$112,484 - $138,984 Duties Elicits recommendations regarding continuous improvement of quality and patient safety programs from staff and develops recommendations for improvement, Encourages reporting of occurrences without fear of reprisal.

Proven experience in hospice & palliative medicine or related fields such as critical care, ICU, emergency medicine, or medical-surgical units.
Excellent communication skills for triage calls, family education sessions, case management coordination, and interdisciplinary collaboration.
