About the role

  • We are seeking a compassionate and experienced Nurse Case Manager to join our team.

Responsibilities

  • Ensure medication adherence and compliance with follow-up appointments through ongoing support and communication.
  • Maintain timely and accurate documentation and communicate effectively with all members of the care team regarding patient status.

Requirements

  • Required: Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Illinois.
  • Minimum of 3 years of nursing experience.
  • Minimum of 2 years of experience with geriatric patients.
  • 2+ years of experience in Case Management or Care Coordination.
  • Experience in post-acute or long-term care settings within the last 5 years.
