- Deliver Patient-Centered Care

- Manage chronic conditions, preventive care, and acute issues within the scope of value-based care principles.
- Utilize evidence-based guidelines to optimize patient outcomes and quality metrics.
- Collaborate with your team and specialists to ensure seamless care delivery.
- Lead care plan discussions and participate in team meetings to review patient progress.
Requirements
- 2-6 years of experience in a primary care setting, preferably managed care, or recent residency graduate.
Nice to have
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Board certification or board eligible in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.

- We offer market-leading compensation.
- Our contracts involve three components: a base salary, a qualitative performance bonus, and beginning in your third year of employment, a clinic performance bonus.
- Your compensation in the first two years should approach $300k, with the opportunity to exceed $400k including your clinic performance bonus.
