About the role
- About the Opportunity: RhynoCare is seeking an experienced Nurse Practitioner (NP) to work in Long Term Care facilities in Barrie, ON.
- You will be hired and onboarded with RhynoCare as an Independent Contractor.
- Provide primary health care services to residents, including assessment, diagnosis, treatment, and follow-up for acute and chronic conditions within full NP scope of practice.
Responsibilities
- Develop, implement, and evaluate person-centered plans of care in collaboration with residents, families, and Substitute Decision Makers (SDMs).
- Select and implement appropriate pharmacological and non-pharmacological interventions to optimize resident outcomes.
- Collaborate and liaise with physicians, Directors of Care, and interdisciplinary teams in care delivery.
Compensation
- $110,000 - 150,000 / year (Depending on experience and assignment)
