Responsibilities
- Develop and revise individualized plans of care based on patient status and evolving needs.
- Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
- Provide counseling and support to patients and caregivers before, during, and after treatment.
Requirements
- Preferably 2+ years of nursing experience, with a focus on geriatrics.
- Nursing degree from an accredited institution.

Compensation
- Pay Strucutre: Paid per visit ($50 weekdays / $55 weekends & holidays).
- Training, meetings, and other hourly activities are paid at $35/hr.
