About the role

- Management of conditions as indicated based on evidence based medicine.
- Provide monitoring and continuity of care between visits.
Responsibilities
- Performs other duties as assigned
- Ability to obtains patient history and perform assessments via interview and examination.
- Maintain and review patient records, charts, and other pertinent information.
Requirements

- Graduated from an APRN/PA program. Required
- The APRN/PA will work in a team environment with collaboration of a physician for assistance in clinical decision-making, case management and evaluation of performance. (Required proficiency)
- Ability to order, conduct and interpret diagnostic studies related to specialty area and visit purpose. (Required proficiency)
- All APRN incumbents are required to maintain Registered Nurse competencies as designated by Federal and State organizations and as outlined in SVH policy. (Required proficiency)
Skills
- Registered Nurse Must hold a RN license with advanced practice certification Required or
- Physician Assistant (PA)
- KSBHA Required
- Additional certifications may be required for certain patient care settings or populations.
- Drug Enforcement Agency (DEA) License is required.
- Basic Life Support
- BLS Required within 90 days of hire.
- Department of Transportation (DOT) Medical Examiner certification is preferred.
Compensation
- Abide by Stormont Vail's Remote Worker Policy and will review and acknowledge the Remote Work Agreement annually.

- Collaboration with physicians in managing acute and long term medical needs of patients.
- Manage medical emergencies.
- Interview and advise patients regarding health and illness prevention.
- Ensures that care delivered to patients is provided according to the Stormont Vail Health mission, strategic plans, and all internal/external regulations, policies and procedures.
- Supports the established care models of Stormont Vail Regional Health Center.
- Document and manage information using electronic medical record and other tools
- Management of health conditions via referral to other healthcare providers or community resources as indicated.
- Counsels and educates patients regarding preventative health, diagnosis, treatment options and resources including referrals.
