Responsibilities
- Create, monitor, review, and update individual service plans every 6 months that are culturally responsive and timely to the needs of the Client.
- Cultivate and maintain a supportive effective working relationship with Client.
- Attend HIV case management meetings/trainings, grant related trainings and other relevant trainings to maintain a comprehensive understanding of HIV and the communities being served.
Requirements
- Must be proficient in MS Word, Excel, and PowerPoint.
- and/or weekend work will be required.
- Must be a self-starter and team player.
- Knowledgeable about and sensitive to the needs persons diagnosed with HIV.
- Good organizational skills
- Well-developed communication skills and team player
- Flexibility with schedule: although typical schedule is Monday-Friday 8am-5pm, some evenings
- services after an assessment of needs is taken.
Nice to have
- Bilingual in Spanish and English preferred (verbal and written), but not required.
Skills
- Must have the ability to read, speak, and translate English and Spanish languages.
- Must be able to problem solve for the clients.
- Must also have the reasoning ability to refer clients to appropriate agencies for community
- A Bachelor's degree in public health, health care administration, public, or business administration,

- Paid-time-off starting at 24 days per year + 10 paid Holidays.
