Responsibilities
- Collaborative Care: Actively partner and communicate with assisted living staff to ensure seamless care coordination and adherence to practice standards.
- Strategic Contribution: Participate in key committees and task forces to lead process improvement, quality assurance, and policy development.

- You will drive our value-based care model forward, providing expert mentorship to a dedicated team of APPs while ensuring clinical excellence for residents in assisted living and memory care environments.

Requirements
- Must be licensed or willing to become licensed in the state of Florida.
- Medical License: Must be licensed or willing to become licensed in the state of Florida.
- Board certification: ABMS or AOA
- Skills: Excellent communication and collaboration skills, with the ability to lead and support a diverse team of APPs.
Nice to have
- Proven experience in clinical practice, geriatrics and chronic disease management preferred, with a strong background in leadership and mentorship.
