About the role

  • JOIN THE PRIME HOME HEALTH TEAM TODAY FT OPENING IN NEW BRAUNFELS As a Registered Nurse for Prime Home Health, you will provide compassionate and skilled nursing care to patients on a Fulltime basis in the comfort of their homes.
  • Your expertise, clinical judgment, and strong interpersonal skills will be crucial in delivering high-quality care and ensuring positive patient outcomes.
  • Regularly reassess patients' conditions, monitor vital signs, and evaluate the effectiveness of interventions, adjusting care plans as necessary.

Responsibilities

  • Conduct comprehensive assessments of patients' physical, emotional, and social needs to develop individualized care plans.
  • Perform skilled procedures such as venipuncture, tracheostomy care, and respiratory therapy.
  • Monitor and manage symptoms, pain, and comfort levels, implementing appropriate interventions to alleviate distress.
  • Maintain timely, accurate and thorough documentation of assessments, care provided, and patient outcomes using Nestmed AI charting tools and Kinnser EHR system.

Nice to have

  • Proficiency in using EHR (Kinnser/WellSky a plus) systems, Nestmed or other AI charting and other apps such as Slack.

Skills

  • Ensure compliance with all regulatory requirements, agency policies, and standards of care.
  • Complete and submit clinical documentation, including OASIS assessments, within specified timeframes.
