About the role
- As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.
- You are a key part of the care team - delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes.
Responsibilities
- Implement and support the plan of care developed by the RN
- Deliver skilled nursing services, including treatments, medication support, and wound care
- Maintain infection control and proper use of equipment and supplies
- Document patient care accurately and timely within the EMR
- Support coordinated care by maintaining consistent communication across the care team
Requirements
- Ability to follow care plans while recognizing and reporting changes in condition
- Basic proficiency with EMR systems
- Ability to work independently while staying connected to a team
- Familiarity with Homecare Homebase (HCHB)
