About the role
- Partnered with Population Health Nurse Practitioner to assist in the delivery of health care and patient care management in a clinical capacity.
- Daily responsibilities of the position are based solely on the needs of the department at any given time.
- Provides directly and through delegation, exceptional care for patients.
Responsibilities
- Maintains knowledge of normal/abnormal parameters that identify patients at risk.
- Performs and follows through in all indicated orders from provider.
Requirements
- Obtains appropriate referrals required.
Nice to have
- LPN license preferred.
- 1 year of experience preferred
- Certification preferred
