About the role
- Responsible for the implementation and ongoing evaluation of the elder's comprehensive plan of care.
- Uses established work systems and processes to facilitate quality of care and quality of life while ensuring ongoing regulatory compliance with all applicable laws and professional standards of practice.
- Supervises Certified Nursing Assistants, RNs and LPNs within 24-hour teams for the assigned group of elders.
Responsibilities
- The RN-Team Advisor directs and supervises the 24-hour care of assigned elders to deliver whole-person care in an innovative elder-directed care model.
- Facilitates effective communication of elder needs and issues to the clinical support team.
- Fosters positive change, professional/program growth and teamwork in an innovative care setting designed to be transformational in the lives of elders and those who care for them.
