About the role
- Reaching out to patients while inpatient to introduce TOC program and communicate post discharge follow up from TOC clinicians
- Scheduling post discharge follow ups (PCP/Specialty)
- Reaching out to hospitals and provider offices to obtain clinical documentation
Responsibilities
- Provide additional operational support for assigned team and manager as designated
- Collaborate internally in clinical rounds to discuss patients at risk for readmission

Skills
- 2+ years of patient facing and care coordination experience within healthcare, managed care, home health, and/or community health setting.
- Experience working in a telemedicine environment.
- Fully bilingual (English/Haitian Creole).
- Managed Care / Health Plan experience
- Thrive in a fast-paced, high-energy, and rapidly growing organization
- Skilled at easing stress and anxiety for patients during difficult times
- Strong communicator, able to effectively interact with various stakeholders including doctors, pharmacies, nurses, and caregivers
- Comfortable working with patients via video, phone, portal
- Entrepreneurial self-starter who takes initiative and thrives as a collaborative team player
- Passionate about helping others improve their health and navigate the healthcare system with compassion, empathy, and warmth
