About the role
- As a Palliative Care Nurse Practitioner, you'll provide compassionate, patient-centered care to individuals living with serious, chronic, or life-limiting illnesses in the comfort of their homes.
- Position Overview The Palliative Care Nurse Practitioner provides comprehensive in-home medical care for patients with serious illnesses.
Responsibilities
- Develop, implement, and adjust individualized care plans focused on symptom management and quality of life.
- Evaluate and manage pain, shortness of breath, nausea, anxiety, fatigue, and other complex symptoms.
- Monitor patient progress and adjust treatment plans in collaboration with physicians and interdisciplinary partners.
- Coordinate care that supports patient goals while helping reduce unnecessary emergency department visits and hospitalizations.
- Lead conversations around advance care planning, goals of care, and advance directives.
- Build meaningful, long-term relationships with patients and families.
Requirements
- Master's or Doctoral degree from an accredited Nurse Practitioner program.
- Ability to work independently while managing a mobile patient caseload.
