About the role

This is an exciting opportunity for a compassionate and skilled Nurse Practitioner (APRN) to provide virtual primary care through our innovative telehealth platform.
As a Primary Care Nurse Practitioner (Routine Care), you'll meet a panel of patients in the first step of their primary care journey with Devoted Medical.
You will also provide that panel with ongoing management of chronic conditions, preventive screenings, and medication management, all through a virtual care model.

Manage medication prescriptions including controlled substances, titrations, and refills, ensuring patient adherence and safety.
You'll conduct 1 hour comprehensive clinical assessments, support chronic condition management, and emphasize preventative and proactive care.

Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings.
Flexibility is required.
Additionally, weekend shifts may be required in the future based on evolving patient care needs.

Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
Experience managing chronic conditions, preventive care, and acute illnesses via telehealth.
Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings, etc.) is a plus.
Comfortable with digital documentation and telemedicine platforms (experience with EHRs and virtual care workflows preferred).
Active BLS certification at time of hire.
Minimum of 6 months of experience working as a Nurse Practitioner in the D2Me program required
