About the role
- This is an exciting opportunity for a compassionate and skilled Nurse Practitioner (APRN) to provide virtual primary care through our innovative telehealth platform.
- As a Primary Care Nurse Practitioner (Routine Care), you'll meet a panel of patients in the first step of their primary care journey with Devoted Medical.
- You will also provide that panel with ongoing management of chronic conditions, preventive screenings, and medication management, all through a virtual care model.
Responsibilities
- Manage medication prescriptions including controlled substances, titrations, and refills, ensuring patient adherence and safety.
- You'll conduct 1 hour comprehensive clinical assessments, support chronic condition management, and emphasize preventative and proactive care.
Requirements
- Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably with experience in telehealth or virtual care settings.
- Flexibility is required.
- Additionally, weekend shifts may be required in the future based on evolving patient care needs.

Skills
- Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
- Experience managing chronic conditions, preventive care, and acute illnesses via telehealth.
- Familiarity with value-based care models, population health management, and quality metrics (HEDIS, STAR ratings, etc.) is a plus.
- Comfortable with digital documentation and telemedicine platforms (experience with EHRs and virtual care workflows preferred).
- Active BLS certification at time of hire.
- Minimum of 6 months of experience working as a Nurse Practitioner in the D2Me program required
