About the role
- The nurse practitioner will be responsible for providing regularly scheduled adult primary and preventive patient care, urgent care services and comprehensive education for patients with diabetes.
- Provides coverage on the phone or in person for health center patients that require urgent care, including, initial assessment, treatment, and triage.
- Respond to phone calls from established health center patients seeking advice or medication refills.
Responsibilities
- Perform chart review of Nurse Practitioners.
- Perform subject to BVMI Quality Assurance protocols.
Nice to have
- Minimum of 5 years experience in a multi-physician or hospital-based primary care practice.
- Bi-lingual in English and Spanish is preferred.
- Experience in diabetes care preferred but not re required (Comprehensive on-the-job training and support will be provided).
