About the role
- This role offers a unique opportunity to advance preventive, geriatric-focused care for seniors in under-resourced communities.
- In this position, you will help bring specialized geriatric expertise into community health centers that traditionally do not have dedicated geriatric services, improving both the level and quality of care delivered to older adults.
- The Medical Assistant plays a critical role in preparing patients for visits, supporting providers, and ensuring comprehensive wellness assessments that focus on prevention, early detection, and chronic condition management.

- High school diploma or equivalent required.
- Knowledge of Medicare preventive services and screenings.
- Fluent Spanish speaker required

- THE OPPORTUNITY: MHN is seeking a dedicated and compassionate Medical Assistant to support Annual Wellness Visits (AWVs) for Medicare patients in community health center settings in the Chicago metropolitan.

- Prepare patients for Medicare Annual Wellness Visits, including intake, documentation, and vital signs.
- Collect and document patient medical history, medications, and health risk assessments in the electronic medical record (EMR).
