About the role
- Respond to emails and phone calls within 2 business days.
- Attend all mandatory Synergy or PRP Program meetings.
- Have a caseload of a minimum of 12 clients
Responsibilities
- Provide Human Resources documents when requested.
- Attempt to contact client at least 3 times in a two-week time period and document attempts in memo section
- Perform rehabilitation activities in the SFS' facility, in clients' homes, and in the community
Requirements
- Complete required job training on site and/or off site.
- Any paperwork corrections must be submitted within three days of the request
- Monthly Billing Summaries must be submitted by 11:59pm on the 3 rd regarding billables provided the month prior.
Nice to have
- Bilingual (Male Preferred) candidates are a plus.
