About the role

  • This role serves as the operational bridge between members, family caregivers, managed care organizations, and Entyre's internal teams to ensure a seamless and compliant care experience.

Responsibilities

  • Home visits: Conduct initial and ongoing home visits to evaluate member needs, develop Entyre service plans based on authorized services, assess caregiver competency, and identify opportunities to improve quality, safety, and member satisfaction.
  • Training & Support: Onboard new caregivers.
  • Support caregiver retention.
  • Act Like Owners: Take initiative and move fast to drive results for families.
  • Support accurate documentation for billing.

Requirements

  • Bachelor's degree appreciated not required
  • Ability to travel (by automobile) within assigned geographic area
  • valid driver's license and automobile insurance is required.

Nice to have

  • AHCCCS, ALTCS, DDD, Medicaid

Skills

  • Full-time hybrid position offering autonomy while utilizing your assessment, care planning and case management skills.

  • Working in partnership with ALTCS health plans, DDD Support Coordinators, physicians, and family caregivers, the Care Manager ensures members receive high-quality Home and Community-Based Services (HCBS) in accordance with authorized service plans while maintaining compliance with AHCCCS, EVV, and Entyre quality standards.
