About the role

As an Addictions Counsellor/Case Manager, you will be integral to the development, implementation, operation, and evaluation of PAARC's Community Concurrent Disorders Program (CCDP).
This role involves processing referrals, conducting comprehensive assessments, and creating evidence-informed care plans, including crisis and communication plans.
Provide ongoing supportive counselling and case management, including assessment, referral, aftercare, and discharge, while effectively managing a defined caseload.

Respond to crisis situations with appropriate interventions and support clients in developing and understanding healthy boundaries.
Able to build and maintain strong relationships with multiple community organizations such as hospitals, social service organizations and other community partners for client intakes, case management and referrals.
This program supports clients who frequently seek help from hospital emergency departments and have addictions, substance use and concurrent disorders.
You will provide short-term transitional case management to clients with a goal of establishing longer-term community supports.

Specified minimum 2 years of relevant experience required.
Valid Ontario Driver's License, insurance, and reliable vehicle required.
Proficiency in non-violent crisis intervention techniques and CPR/First Aid.
Demonstrated knowledge of harm reduction and an alignment with PAARC's philosophy of substance abuse.

Strong evidence-based, trauma-informed counselling skills essential.
Proficiency in prevalent languages in the Region of Peel and French considered a strong asset.
Transportation: Valid Ontario Driver's License, insurance, and reliable vehicle required.
Working Conditions Community-based settings (80-90%) and in-office (10-20%) 35-hour work week; Mon - Fri - Fri 8:30 am- 4:30 pm.
May be required to work after hours (up until 8:00pm) to accommodate client needs.
Required travel to clients homes and community facilities.
