About the role
- As an Addictions Counsellor/Case Manager, you will be integral to the development, implementation, operation, and evaluation of PAARC's Community Concurrent Disorders Program (CCDP).
- This role involves processing referrals, conducting comprehensive assessments, and creating evidence-informed care plans, including crisis and communication plans.
- Provide ongoing supportive counselling and case management, including assessment, referral, aftercare, and discharge, while effectively managing a defined caseload.
Responsibilities
- Respond to crisis situations with appropriate interventions and support clients in developing and understanding healthy boundaries.
- Able to build and maintain strong relationships with multiple community organizations such as hospitals, social service organizations and other community partners for client intakes, case management and referrals.
- This program supports clients who frequently seek help from hospital emergency departments and have addictions, substance use and concurrent disorders.
- You will provide short-term transitional case management to clients with a goal of establishing longer-term community supports.
Requirements
- Specified minimum 2 years of relevant experience required.
- Valid Ontario Driver's License, insurance, and reliable vehicle required.
- Proficiency in non-violent crisis intervention techniques and CPR/First Aid.
- Demonstrated knowledge of harm reduction and an alignment with PAARC's philosophy of substance abuse.
Nice to have
- Current CPR and First Aid required
- GAIN Q3 MI ONT Certification preferred.
- Certifications: Current CPR and First Aid required
Skills
- Strong evidence-based, trauma-informed counselling skills essential.
- Proficiency in prevalent languages in the Region of Peel and French considered a strong asset.
- Transportation: Valid Ontario Driver's License, insurance, and reliable vehicle required.
- Working Conditions Community-based settings (80-90%) and in-office (10-20%) 35-hour work week; Mon - Fri - Fri 8:30 am- 4:30 pm.
- May be required to work after hours (up until 8:00pm) to accommodate client needs.
- Required travel to clients homes and community facilities.
