About the role

  • The Care Coordinator facilitates progression of care and monitors the patient's progress to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective.
  • Care Coordination improves access to services to improves outcomes and reduce over-utilization of resources.

  • Responsible for maintaining strict confidentiality of all verbal and written information and documents.

Requirements

  • Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity.
  • Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress.
  • Familiarity with and appropriate use of community resources.
  • Lifting at least 10 pounds is required.
  • Must be able to work with a moderate level of noise.

Nice to have

  • Graduate of accredited school of Nursing, Baccalaureate degree preferred.
  • 3-5 years of hospital adult acute care nursing experience preferred.
  • CCM, CMAC, or ACM preferred.
