About the role

  • We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management, pain control, and quality of life for individuals with serious or chronic conditions.

Responsibilities

  • Develop and manage inpatient consultation emphasizing comfort, symptom control, and patient goals
  • Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams
  • Consistent schedule designed to support work-life balance

Requirements

  • Ability to work independently while coordinating care across settings
  • Must have driver's license and reliable source of transportation and insurance.
  • Active California Nurse Practitioner or Physician Assistant license
  • Strong interdisciplinary team leadership skills
  • Excellent time management and organizational abilities
  • Comfortable with home visits and community driving
  • Strong communication skills with patients, families, and care teams

Nice to have

  • Credentialed at either Orange County, LA County, Long Beach, Temecula Valley, Riverside, San Bernardino, San Diego County area hospitals

Compensation

  • $85 - $120 USD
