About the role
- We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management, pain control, and quality of life for individuals with serious or chronic conditions.
Responsibilities
- Develop and manage inpatient consultation emphasizing comfort, symptom control, and patient goals
- Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams
- Consistent schedule designed to support work-life balance
Requirements
- Ability to work independently while coordinating care across settings
- Must have driver's license and reliable source of transportation and insurance.
- Active California Nurse Practitioner or Physician Assistant license
- Strong interdisciplinary team leadership skills
- Excellent time management and organizational abilities
- Comfortable with home visits and community driving
- Strong communication skills with patients, families, and care teams
Nice to have
- Credentialed at either Orange County, LA County, Long Beach, Temecula Valley, Riverside, San Bernardino, San Diego County area hospitals

- $85 - $120 USD
