About the role
- Know each resident and their family including a psychosocial profile of the resident and an understanding of the family members and dynamics.
- Participates in care planning for resident needs.
Responsibilities
- Lead the Grievance Committee, hearing any resident concerns and following up on them timely.
- Support and encourage social work interns as needed
- Understands the process for Medicaid Waiver and helps support transitions of care
- Weekly support to each level of care in Independent Living, Assisted Living and Nursing.
- We offer different lifestyle options for seniors through independent living, assisted living, skilled nursing, rehab, memory support, respite care, in-home care, and hospice services.
Requirements
- Education: Post-secondary education preferred
- Licensure: MI Licensed Social Worker
