- Optima is currently seeking a Behavioral Health Coordinator to join our team!
Responsibilities
- Develop and monitor individualized care plans.
- Facilitate referrals and support appointment and treatment adherence.
- Support transitions of care, including hospital admissions and discharges.
- Maintain timely, accurate, and compliant documentation.
- Collaborate with interdisciplinary teams to improve patient outcomes.
Requirements
- Master's Degree in any field
Nice to have
- Strong organizational, documentation, and time-management skills.
- Ability to utilize evidence-based engagement techniques such as motivational interviewing, active listening, behavioral activation, grounding techniques, safety planning, and problem-solving strategies.
- Commitment to ongoing professional growth and development.
- Experience working with Medicaid, Medicare, or managed care populations.
- BHT certification (if applicable).
- Bilingual or multilingual capabilities.
