About the role
- Responsible for assisting clients and families with social and financial issues related to continuing care admissions or return to the community.
- Collaborating with the Community Care Coordinator, Transition Services to assist with the development of the discharge plan.
- Communicating with the client and family about progress toward the appropriate discharge location.
Requirements
- Graduation from a recognized degree program in Social Work or equivalent program Registered with Alberta Association of Social Workers;
- Minimum of 2 years experience in a combination of clinical and continuing care roles;
- Driver's License
Nice to have
- Gerontology experience is preferred
