About the role
- Provide Coordinated Specialty Care (CSC) model to families and consumers with severe mental illness, serious emotional disturbance, and other team members' consumers and families.
Responsibilities
- Engage with consumers monthly to assess needs, develop and address care plan goals, and coordinate care.
- Collaborate with consumers' support system and identify / increase supports when needed.
- Develop and maintain collaborative relationships across departments.
- Develop and maintain collaborative relationships with community partners.
- Perform other duties as assigned
Compensation
- $41,000 (base pay) with additional incentive for language skills and behavioral health experience.
- Responsible for attending all mandatory in-services, continuing education and annual health requirements
