About the role
- The Primary Care Registered Nurse (PCRN) is a key member of the interprofessional healthcare team.
- Counsel and guide patients on symptom management, health maintenance, and rehabilitation strategies, as well as risk factors and lifestyle changes, and involve them in decision-making and self-management.
- Provide and coordinate screening for health issues such as diabetes, hypertension, cholesterol, cardiovascular health, mental health, and cognitive function.
Responsibilities
- As a Primary Care Network, we work alongside hundreds of family doctors (our members) to support the patient-doctor relationship and provide programs and services at no cost to doctor or patient.
- Offer training and support for other patient care team members and assist in their professional development.
Requirements
- A bachelor's degree in nursing is required.
- Experience with successful QI initiatives in the workplace is an asset.
- Registration: Active registration with the College of Registered Nurses of Alberta (CRNA) in good standing is required.
- Technical: Proficiency in the Microsoft Office Suite (Word, Excel, Outlook, PowerPoint, Teams) and EMR systems.
- Background check: A clear Criminal Background check with Vulnerable Sector check is required to start.

- Participate in the identification of new service requirements and addresses deficiencies in current services for the benefit of patients.
