About the role

  • In this role, you will coordinate social and logistical aspects of care, spending time connecting with patients and caregivers by phone, video, and occasionally in the home.

Responsibilities

  • Support development of patients' care plans and participate in case conferences to ensure quality care delivery.
  • Own structured chart preparation to ensure providers are visit-ready, including accurate contact, insurance, and care team information.
  • Comfort with Technology. You're comfortable using Slack, Zoom, the Google Suite, and EHR systems to ensure accurate appointment scheduling and management and support patients virtually.

Compensation

  • $50,000 - $60,000 USD

  • Document social interventions, tasks, and care coordination activities in the electronic health record in a timely and standardized format.
  • You have at least 3 years of experience as a Medical Assistant, CNA, EMT, or in a similar clinical or care coordination setting.
  • Oncology experience is a bonus.
