About the role
- Conducts outbound patient consultations and comprehensive medication reviews while ensuring compliance with clinical guidelines and quality measures.
- Provides detailed medication counseling and statin-related consultations aligned with American Heart Association (AHA) guidelines.
- Reviews patient medication profiles, diagnoses, and treatment plans to identify opportunities for intervention and optimization.
Responsibilities
- Perform Medication Therapy Management consultations
- Document pharmacist recommendations accurately and efficiently
- Support Medicare Part D Star Measures initiatives including SUPD, SPC, and CMR
Requirements
- Required License:
- Minimum 4 years of experience as a Clinical MTM Pharmacist
- Experience conducting Medication Therapy Management (MTM) services
- Experience completing Comprehensive Medication Reviews (CMRs)
- Familiarity with Medicare Part D Star Measures
- Experience supporting SUPD, SPC, and CMR initiatives
- Ability to conduct high-volume outbound patient calls
Nice to have
- Prior management or leadership experience
- Salesforce experience
- Experience working in quality, adherence, or Star Ratings programs

- $114k-$119k
