About the role
- The Housing Case Manager - Rapid Rehousing TAY will provide comprehensive case management to TAY youth placed into housing via San Francisco's Coordinated Entry.
- The Housing Case Manager - Rapid Rehousing TAY will assist clients with securing needed documentation to move into housing, provide linkages and referrals to resources etc.
- They will also provide wrap-around case management services with a harm reduction model to ensure the clients long-term housing retention and improved well-being.
Responsibilities
- 3rd Street Youth Center & Clinic also manages a number of successful Transitional Age Youth (TAY) housing stabilization programs and one of the city's few Youth Access Points.
