- JOB DESCRIPTION: SUMMARY OF POSITION Provides therapeutic interventions aimed at fostering personal growth and resilience in clients with behavioral health diagnoses.
Responsibilities
- Develops individualized care plans that are person centered, family centered, and service centered.
- Provides community support, care coordination/case management, and crisis intervention services.

Requirements
- Master's Degree in Human Services field, licensure eligibility required

- Maintains documentation of services provided and submits for review as required.

Skills
- Knowledge of community geographical awareness and of community and agency resources.
- Skill in interviewing, assessment, and counseling.
- Proficiency in customer service techniques, demonstrated through effective resolution of client issues.
- Advanced problem-solving skills, evidenced by successful project outcomes.
- Familiarity with digital communication systems, including Electronic Health Record for managing client interactions.
- Ability to conduct thorough assessments and form diagnoses, utilizing standardized tools and methodologies.
- Expertise in virtual communication platforms, ensuring clear and effective remote interactions.
- Strong analytical skills, applied in evaluating individual needs and resources.
Compensation
- $48,300.00--$67,700.00
