About the role
- Clearway Health is seeking a Clinical Pharmacist!
- This position e nsures optimal patient outcomes related to chronic and specialty drug-related disease states.
- Your tasks include preventing or resolving medication-related issues and optimizing drug therapy.
Responsibilities
- Collaborate with referring physicians to optimize pharmacotherapy according to standard care guidelines for optimal patient outcomes.
- Identify and implement cost-saving and containment programs.
- Implement quality improvement initiatives in the ambulatory setting.
Requirements

- A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
- Completion of PGY-1 Pharmacy Residency or 3+ years working as a clinical p harmacist in direct patient care is required.
- An active Pharmacist licensure (in good standing) to practice pharmacy in the state in which you are applying in or and/or the ability to acquire this at the time of hire is required.
- Must possess or be able to obtain a State Board of Pharmacy Preceptor's license by the current state you are applying to within one year of hire.
- Up to 10% travel may be required.
Nice to have
- 2+ years of work experience in ambulatory care pharmacy setting post PGY1 training or 3+ years post PharmD is preferred.
- Completion of a PGY-2 Ambulatory Care or Specialty Pharmacy R esidency/Fellowship) is preferred.
Skills
- Demonstrated expertise in medications, drug interactions, medical procedures, therapies, diagnoses, and disease states is essential.
- Organized work approach with a proficiency in maintaining accurate documentation.
- Strong verbal and written communication skills; proficiency in additional languages is advantageous.
- Self-directed with strong self-management abilities; capable of identifying and addressing issues independently.
- Proven track record in process improvement and a keen eye for detail and precision.
- Proficiency in Microsoft Office programs (e.g., Word, Excel, Outlook) and web browsers.
Compensation
- The current annual compensation range for this position is $146,000 - $182,000.
